In theory misses are also cached, with the intent of not trying to
retrieve again an avatar it's known to fail until some long time has
passed.
It could be simply a bug on that caching, although, the underlying
infrastructure serving libravatar has also been changed since plugin
creation, so that's another factor to consider.
Just have to find the time to investigate, despite being confined at home
24 hours a day and working remotely I have less free time than ever :-/
