Add more information to input dialog's title

I use KeePassXC to manage passwords and I like to keep them in memory only, so
I will input them every time when I open claws and the IMAP server needs
credentials. KeePassXC can read the window's title to select correct username
and password. So, I made a very tiny patch to have the username and the server
in the password input dialog. This makes it a little bit more convenient to
input the password.
