Years and years ago, I had a similar problem with the CM.
Eventually somebody mentioned that it could a drive problem.

I would suggest that you run smartctl:

> sudo smartctl -a /dev/sda (or whatever drive it is)

And it was a drive problem. My drive was in the process of dying and
had to replace. Never had the problem since...
