OTOH, you can do it with regular expressions, ('use regexp' checkbox).

For your 2 examples:

1: s metabol & s sugar | s metabol & s carboh

-------8<--------8<--------8<--------8<--------

subject matchcase "metabol" & subject regexpcase "(sugar|carboh)"

------->8-------->8-------->8-------->8--------

or, if you wanted to match whole words only:

-------8<--------8<--------8<--------8<--------

subject regexpcase "\\bmetabol\\b" & subject regexpcase "\\b(sugar|carboh)\\b"

------->8-------->8-------->8-------->8--------

2: s sugar | s carboh & s metabol

subject matchcase "sugar" & subject regexpcase "(carbol)|(metabol)"
