Upon further testing and reading the source, it appears that this isn't a
general problem with second-level menus.  With some of them you can set
shortcuts, and with some of them you can't.  From looking at compose.c, it
looks like there is simply no shortcut identifier defined for the commands
under Spelling/Options.
